Registering at Goodwood Court Medical Centre - Permanent
patients

If you are in the area for less than 6 months, permanent registration
may not be open to you - if you need medical help and are a short-term
resident, you should approach any medical surgery for assistance.

While we are pleased to be able to provide care for you, the data
requirements now enforced upon us as medical practitioners require a
considerable amount of information.
Regrettably it may take up to 12 weeks or more to transfer your
previous medical records and these are usually in a form which requires
considerable collation and assimilation.
We have therefore introduced a requirement that patients registering
with this practice go through a mini-health check which may include an
ECG, an assessment of your Body Mass and Fat content, a breathing test
and an assessment of your bone density. We also require knowledge of
your cholesterol and sugar metabolism and a full medical history.
You will be asked to complete documentation fully, then to see our
Healthcare Assistant for the above assessments; to attend the phlebotomy
clinic for blood tests and also to see one of the doctors if you are on
regular medication - all this before we can confirm full registration.
Hence:
1: Complete the questionnaire and registration
form and give in to reception. You will be asked for confirmatory
photographic identification (passport, driving licence etc) and also
confirmation of address and intention to remain in UK for more than 6
months. If from
non-EEC you will also be asked for sight of your VISA which allows you
to stay in the UK
2: Book an appointment for your "Healthscreen" by telephoning 0844 477
0925 – this will be undertaken at Goodwood Court Medical Centre
